Human error causes around 75% of accidents and fatalities in the maritime sector. According to Allianz Global Corporate & Specialty, at least three quarters of these events occur due to human circumstances that can be foreseen.

In recent years there have been multiple incidents in which many containers have fallen overboard, leaving thousands of dollars in the middle of the sea. Most of them due to causes that could be avoided and that we present below:

1. Stowage failure

Due to the speed of the procedures and the high demand, on many occasions the stowage is not carried out properly. Therefore, the higher the intermodal equipment tower, the more instability the ship may experience in the middle of the route. Especially if the container stack is overloaded.

2. Overworked crews

As a result of the increase in demand and the reduction of the qualified workforce for the processes, it is usually more and more difficult to supervise that each procedure respects the indications and requirements.

Faced with this scenario, Neil Wiggins, managing director of Independent Vessel Operations Services, assured that «it is increasingly difficult for crews to check each bar and screw effectively».

3. External deficiencies due to lack of communication or knowledge

Beyond the faults of the crew, the faults of other workers also have an influence. Among them, land and port personnel, inspectors or members of shipping companies.

An investigation by the University of the Basque Country stated that, among the causes of these accidents due to human failure, other factors must be taken into account. These include «the errors made by the people who work around the ships», because «if the pilot who assists the ships in the ports gives you bad advice and an accident occurs, the responsibility does not lie with the crew».
